Before Your Appointment

Please download and print our patient information forms found below.  Please bring your completed forms with you to your appointment; there is no need to mail them to us.

If you are filing a Worker’s Compensation claim, please complete the Worker's Compensation Claim Form.  Please see the Worker's Compensation Page for additional information.

Your physician will want to review any tests or imaging studies that you have had that are related to your problem.  In order to avoid delays during your visit, or even a second appointment, please make arrangements to obtain your previous tests, X-rays, MRI’s, nerve conduction studies, lab results, etc.  You can use the Authorization for Release of Patient Information form to request the copies from the facility that performed the testing.  (Please allow enough time from the date of your request – it may be quicker to pick up the records and bring them in to your appointment, rather than having them mailed to us. During Your Appointment

At your visit, your doctor will obtain a complete history of your problem and will review any films or tests you have brought with you.  He may also order additional x-rays or tests in order to evaluate your problem.  Please allow at least one hour for your appointment.  If you would prefer to return on a different day for testing, please let the doctor know.  You can find more information about the testing performed in our office on our Services page.

 

After You Meet With Your Doctor

We will perform all needed casting and splinting here in our office at the time of your appointment.  If you need an injection, you can choose to have it performed on the same day as your visit, or you can return on a different day.  Sometimes the doctor will order tests outside of our office (EMG, MRI, blood work, etc.)  We will assist you with scheduling these tests.  You will then return to see the doctor to review the results when they are available.  Surgery is scheduled on an elective, urgent or emergent basis.  The doctor and his secretary will work with you to find a day and facility that work with your schedule, as well as with your insurance requirements.
