Your physician will want to review any tests or imaging studies that you have had that are related to your problem. In order to avoid delays during your visit, or even a second appointment, please make arrangements to obtain your previous tests, X-rays, MRI’s, nerve conduction studies, lab results, etc. You can use the Authorization for Release of Patient Information form to request the copies from the facility that performed the testing. (Please allow enough time from the date of your request – it may be quicker to pick up the records and bring them in to your appointment, rather than having them mailed to us. Written reports can be faxed to: (414)-453-7420)
During Your AppointmentAt your visit, your doctor will obtain a complete history of your problem and will review any films or tests you have brought with you. He may also order additional x-rays or tests in order to evaluate your problem. Please allow at least one hour for your appointment. If you would prefer to return on a different day for testing, please let the doctor know. You can find more information about the testing performed in our office on our Services page.
